Participant Commitment Agreement

Please read this commitment statement.

YES! I want to participate in the World Impact Los Angeles Golf Marathon. I believe I can reach the pledge goal of $25 per hole (or total $2500), and hereby commit to put forth the ultimate effort to do so.

YES! I understand that I am to have my sponsorship campaign in full swing within the first week after making my commitment.

YES! I have read the Medical Information & Waiver of Liability and fully agree to it's terms.

YES! I am sponsoring myself at $1.00 per hole ($100 registration fee) for this effort.

Medical Information & Waiver of Liability

I recognize that there is an element of risk in any outdoor sport or activity. Understanding the inherent risks, dangers, and rigors involved in the above-mentioned golf event, I certify that I am fully capable of participating in this golf event.

I hereby assume all risks involved in playing in the above-mentioned golf event. I and my heirs, successors, and assigns will hold the host organization, World Impact, Inc., and the management and owners of the host golf course harmless form any and all liabilities, action, causes of action, debts and claims resulting from the above-mentioned golf event.
